Form IMM 5484 provides a checklist for the documents required when submitting an application for a temporary resident visa.
For minors
Minors travelling alone must carry information concerning the person who will be responsible. If the child is the subject of a custody order or is travelling with one parent, proof of custody or the other parent’s consent for the trip must also be provided. It is recommended that minors travelling without their parents have a letter of permission to travel from the non-accompanying parent(s).
